Under the general supervision of the Rehab Therapy supervisor at the Rehab Center at Mercy Health Center, and according to departmental policies and procedures regarding referral, standards of treatment, performs objective assessments, as well as comprehensive individualized treatment programs for adult and geriatric patients. Responsible for direct supervision to patients, indirect supervision of rehabilitation technicians. General supervision of PT assistants in accordance with Oklahama Practice Act. What You'll Do

As a Physical Therapist, you will:

Evaluate and Develop Individualized Care Plans

  • Perform comprehensive patient evaluations using appropriate assessment tools and clinical judgment.
  • Collaborate with patients and caregivers to establish meaningful, functional goals.
  • Develop personalized treatment plans with measurable outcomes and defined timelines.
  • Coordinate care with physicians, nurses, therapists, and interdisciplinary team members.
  • Document evaluations, plans of care, treatment goals, and patient progress accurately and timely.

Deliver Exceptional Patient Care

  • Administer therapeutic exercises, modalities, and skilled treatment interventions.
  • Monitor patient progress and modify treatment plans as needed.
  • Educate patients and caregivers on treatment strategies, home exercise programs, and safety precautions.
  • Utilize proper infection prevention and patient safety practices.
  • Maintain productivity standards while providing high-quality, individualized care.

Support Positive Patient Outcomes

  • Communicate effectively with physicians and other healthcare professionals.
  • Participate in interdisciplinary care planning and patient conferences.
  • Ensure patients and caregivers are prepared for successful discharge and continued recovery at home.
  • Provide ongoing progress assessments and outcome measurements.

Maintain Clinical Documentation & Compliance

  • Complete daily treatment notes, progress reports, interim summaries, and discharge documentation.
  • Accurately record patient charges and required rehabilitation outcome measures.
  • Document patient education and any changes to treatment plans promptly.

Contribute to Professional Excellence

  • Participate in department meetings, training programs, and quality improvement initiatives.
  • Maintain required licensure, certifications, and continuing education.
  • Assist with onboarding, mentoring, and clinical education for students and new team members when needed.

Qualifications

Required

  • Current Oklahoma Physical Therapist License (or license eligible)
  • Bachelor's, Master's, or Doctorate Degree in Physical Therapy
  • Current BLS Certification
  • Strong clinical assessment, treatment planning, and patient education skills
  • Commitment to compassionate, patient-centered care

Preferred

  • 6 months to 1 year of Physical Therapy experience in a rehabilitation setting
  • Experience treating adult and geriatric patient populations
  • Experience in inpatient rehabilitation or multidisciplinary care environments

Our Mercy health system was founded by the Sisters of Mercy in 1986. But our heritage goes back more than 185 years. It began with an Irish woman named Catherine McAuley, who wanted to help the poor women and children of Dublin. Though Catherine had a modest upbringing, she received an unexpected inheritance that allowed her to fulfill her dreams. In 1827, she opened the first House of Mercy in Dublin, intending to teach skills to poor women and educate children. Many volunteers came to help.
