Physical Therapist in home health care, you'll design individualized care plans, provide hands-on therapy, and collaborate with a supportive team, all with scheduling flexibility that fits your lifestyle.

Key Responsibilities- Develop and update individualized care plans
- Provide physical therapy services per physician's plan of care
- Assess functional status, mobility, and home environment
- Educate patients and families on therapy techniques and equipment
- Maintain accurate documentation and communicate progress to physicians
Requirements- Licensed Physical Therapist in the State of Colorado
- Degree from a CAPTE-accredited program; passed state-approved exam
- Minimum 2 years' experience (home health preferred)
- Current CPR certification
- Valid driver's license, insurance, and reliable transportation
Competitive Benefits
Salary: Per-Visit Schedule -Range from: $90,000-$110,000.00
