    ABOUT

    In Hazel Park, our mission is to create a supportive and enriching community for special education professionals and clinicians, which will, in turn, lead to positive outcomes for school districts and their students.

    POSITION OVERVIEW

    The School Physical Therapist will be assessing students' physical development level; providing appropriate treatment to meet individualized student objectives; providing recommendations for program development and student placement; and provide direction to Physical Therapy Assistants. We're searching for a PT who is eager to collaborate with a multidisciplinary team including OTs, SLPs, and other ancillary staff. As a Physical Therapist with Hazel Park, you will work in collaboration with teachers, parents, administrators, and other professionals to strengthen connections between home and school, and ultimately creating an effective educational environment.

    DUTIES

    • Assesses students' functional capabilities and home and/or classroom environment for the purpose of determining a student's functional level and developing recommendations and/or placement.
    • Collaborates with the school team to develop Individualized Education Programs and Section 504 Plans to address the students' unique needs.
    • Consults with teachers, parents, other personnel, and/or outside professionals for the purpose of providing requested information, developing plans for services, and/or making recommendations.
    • Coordinates with community agencies including medical, judicial, social service, and mental health services for the purpose of determining appropriate treatments, etc. to meet the needs of specific students.
    • Develops treatment plans, interventions, and/or educational materials for the purpose of meeting individualized student objectives and ensuring compliance with regulatory requirements.
    • Participates in meetings, workshops, and seminars as assigned for the purpose of conveying and/or gathering information required to perform functions.
    • Prepares a wide variety of written materials (e.g. evaluations, correspondence, memos, treatment plans, reports, etc.) for the purpose of documenting activities, providing written reference, and/or conveying information.
    • Research resources and methods (e.g. intervention and treatment techniques, assessment tools and methods, community resources, etc.) for the purpose of determining the appropriate approach for addressing students' functional goals.

    REQUIREMENTS

    • Required Licensure in the state of Michigan (LARA)
    • Experience working with children of all ages and with a variety of needs
    • A passion for working with children
    • The ability to "connect" with staff, children, and parents
    • Exceptional written and oral communication skills
    • Creative and flexible
    • Access to personal transportation

    COMPENSATION

    • In accordance with the HPEA Salary Schedule
    • Position receives a split of Center & Non-Center pay

    Reports to Executive Director of Student Services
