    Physical Therapist (PT) Home Health

    Albuquerque, NM and surrounding areas

    Full-Time | W2 | Direct Hire

    Its a full-time PT role with one of the nations most respected post-acute providers, where youre treated like a clinician, not a number. With strong leadership, well-defined support, and no micromanagement youll have the autonomy, flexibility, and resources to do the work you trained for.

    Our partner, Advanced Health Care, has a national footprint but their Albuquerque team operates with the tight-knit culture of a mission-driven, therapist-led agency. Youll receive 1:1 mentorship, clinical autonomy, and a sustainable caseload designed to prevent burnout and encourage growth.

    Compensation & Benefits:

    • Base Salary: $90,000 $105,000/year (DOE)

    • Mileage Reimbursement

    • Schedule: MondayFriday, No Weekends or On-Call

    What Youll Do:

    • Conduct in-home physical therapy assessments and create customized care plans

    • Help patients regain independence and mobility in a home setting

    • Communicate progress and collaborate with nurses, physicians, and other disciplines

    • Ensure documentation is timely, compliant, and defensible

    • Empower families with education and resources for long-term success

    Requirements:

    • Active PT license in New Mexico (or eligibility)

    • Minimum 1 year of direct home health experience (strongly preferred)

    • Commitment to evidence-based, patient-first care

    • Strong communication and organizational skills

    • Reliable transportation
