Pride Health is hiring a Physical Therapist for the Beckley, WV location to support our client in a Healthcare Setting.

Full-Time Contract | 40 Hours/Week

Monday-Friday | 8:00 AM - 4:30 PM

Pay -$44 to $48/ hr on W2

Key Responsibilities

  • Conduct comprehensive patient evaluations and initiate Plans of Care (POC) and OASIS assessments.
  • Develop and implement individualized treatment plans based on patient needs and physician orders.
  • Deliver skilled physical therapy services in the home health setting.
  • Monitor patient progress and modify treatment plans as appropriate.
  • Collaborate with physicians, nurses, PTAs, caregivers, and interdisciplinary team members to coordinate care.
  • Supervise Physical Therapist Assistants (PTAs) in accordance with state regulations.
  • Maintain accurate, timely, and compliant documentation.
  • Educate patients and caregivers on treatment plans, home exercise programs, safety, and functional mobility.
  • Ensure high-quality patient care and adherence to agency and regulatory standards.

Required Qualifications

  • Active Physical Therapist License
  • Current Physical Therapist licensure in the state of practice
  • Valid Driver's License and reliable transportation
  • Ability to travel throughout assigned coverage areas

Preferred Qualifications

  • Home Health Experience
  • OASIS Documentation Experience
  • Homecare Homebase (HCHB) Experience
  • Strong communication and patient education skills
  • Ability to work independently and manage a home health caseload
