Physical Therapist (PT) – Home Health

 
Location: Charlotte, NC – Union County or East Mecklenburg County
Contract: 13 Weeks Contract
Schedule: Monday–Friday, 8:00 AM–4:30 PM
Mileage: Reimbursed between patient visits
Pay Range: $2617.00 - $2717.00/Week
Dress Code: Medical business casual or scrubs
 
Position Summary
We are seeking an experienced Physical Therapist (PT) to provide skilled physical therapy services to patients in their homes throughout Union County or East Mecklenburg County.
This is a visit-based Home Health position. The PT will travel to multiple patients' homes during the workday, evaluate their physical needs, develop treatment plans, provide therapy, and help patients improve mobility, strength, and independence.
 
Key Responsibilities
  • Provide skilled physical therapy services to Home Health patients in their homes.
  • Evaluate patients and develop individualized treatment plans.
  • Provide therapeutic exercises, mobility training, and functional rehabilitation.
  • Educate patients and caregivers on exercises, safety, and use of assistive devices.
  • Monitor patient progress and update the Plan of Care as needed.
  • Maintain accurate and timely clinical documentation.
  • Coordinate care with physicians and the interdisciplinary healthcare team.
  • Follow Home Health, infection-control, and patient-safety standards.
  • Travel locally between patient visits.
 
Required Qualifications
  • Current, active North Carolina Physical Therapist (PT) license.
  • Previous Physical Therapy experience.
  • Ability to provide services independently in patients' homes.
  • Reliable transportation and ability to travel throughout the assigned coverage area.
  • Strong communication, organization, and clinical assessment skills.
 
Preferred Qualifications
  • Home Health experience preferred.
  • Homecare Homebase (HCHB) experience strongly preferred.
  • Experience working with a Home Health caseload.
