Pride Health is actively hiring a Physical Therapist (PT) to join our client's growing Home Health team.

This is an excellent opportunity for experienced therapists as well as new graduates looking to start their careers in home health.

Freshers are welcome! Candidates with no prior experience are encouraged to apply. Training will be provided.

Location: Cullman, AL, 35055

Coverage Area: Cullman County and Surrounding Areas, Alabama

Compensation:

Local Pay: $55.00 - $62.00/hour (W-2)

Travel Pay Package: Up to $2,700/week gross (includes weekly non-taxable stipend)*

Schedule:

Guaranteed Hours: 40 hours/week

Weekend Rotation: Required

Mileage Reimbursement: Yes (between patient visits)

Key Responsibilities:

  • Conduct comprehensive physical therapy evaluations in patients' homes.
  • Develop individualized treatment plans based on physician orders and patient needs.
  • Provide skilled interventions including therapeutic exercises, gait and balance training, transfer training, pain management, and fall prevention.
  • Recommend and educate patients on assistive devices and home exercise programs.
  • Collaborate with physicians, nurses, occupational therapists, speech therapists, social workers, and other healthcare professionals.
  • Maintain accurate and timely documentation, including evaluations, reassessments, recertifications, and discharge summaries.
  • Ensure compliance with Medicare, state, federal, and agency regulations.
  • Monitor patient progress, modify treatment plans as needed, and provide discharge planning and caregiver education.

Required Qualifications:

  • Bachelor's Degree (or higher) in Physical Therapy from a CAPTE-accredited program.
  • Current, unrestricted Physical Therapist (PT) license in the state of practice.
  • Current CPR/BLS Certification.
  • Valid driver's license, reliable transportation, and current auto insurance.
  • Ability to travel throughout the assigned service area.

Preferred Qualifications:

One year of clinical experience as a licensed Physical Therapist.

Previous Home Health or community-based care experience.

Experience with OASIS documentation.

Knowledge of documenting medical necessity, homebound status, and therapy-specific skilled interventions.
