Pride Health is actively hiring a Physical Therapist to join our client's Home Health Program in a growing care team serving patients.

Job Detail:

Job Title: Physical Therapist - Home Health

Location: Elizabethtown, KY [Coverage Areas: Hardin County, Meade County, and Breckinridge County, Kentucky]

Schedule: Monday - Friday, 8:00 AM - 4:30 PM

Duration: 13+ Weeks

Local Pay: $55.00 - $60.00/hour (W-2) Plus Mileage reimbursement

Travel Pay Package: Up to $2,600/week gross (includes weekly non-taxable stipend) * Plus Mileage reimbursement

  • Negotiable & Pay offered is based on experience, expertise, credentialing, and education.

Weekly Hour Guarantee: 40 hours

On-Call/Weekend Requirement: None

Mileage Reimbursement: Yes (from first patient to last patient)

Key Responsibilities:

  • Evaluate patients' functional status, physical therapy needs, and the status of body systems as required for CMS documentation.
  • Consult with physicians and other members of the healthcare team in developing and implementing the patient's therapy plan of care.
  • Develop individualized physical therapy plans of care in accordance with state-specific guidelines and the patient's clinical needs.
  • Provide skilled physical therapy services within the scope of practice defined by Kentucky state law and in accordance with the established plan of care.
  • Perform therapeutic interventions using accepted standards of practice and evidence-based techniques to relieve pain, restore or maintain function, improve mobility, and maximize patient independence.
  • Monitor and document the patient's response to treatment, progress toward goals, and any changes in condition.
  • Report significant changes in the patient's condition or response to treatment to the nurse supervisor and/or physician.
  • Complete patient evaluations and communicate evaluation outcomes, goals, and anticipated frequency of care to the nurse supervisor within 24 hours of the evaluation.
  • Complete accurate progress notes for each patient visit and submit documentation according to agency policy.
  • Educate patients, family members, and caregivers regarding the plan of care, functional limitations, therapeutic interventions, safety, and progress toward established goals.
  • Collaborate with other members of the healthcare team to coordinate and deliver comprehensive patient care.
  • Provide services throughout the assigned service area and nearby agency locations as needed.
  • Provide supervision to Physical Therapist Assistants (PTAs) in accordance with agency policy and Kentucky state regulations.
  • Supervise home health aides when applicable and permitted by state regulations and agency policy.
  • Prepare written discharge summaries and/or patient condition summary reports at least every 60 days, as required.
  • Participate in staff conferences, committees, and agency meetings as requested.
  • Provide in-service education and clinical support as requested by agency leadership.
  • Maintain and enhance clinical knowledge and skills through continuing education and professional development.
  • Maintain current state licensure and remain up to date with applicable standards of care and regulatory requirements.
  • Adhere to established Therapy Standards of Practice and all applicable agency policies and procedures.
  • Maintain schedule flexibility to meet patient care needs, including participation in weekend rotations and potential observed-holiday coverage when required.
  • Perform other duties as assigned.

Required Qualification/Certification

  • Graduate of an accredited Physical Therapy program
  • Active Physical Therapist license in the state of assignment
  • At least 1 year of home health experience and experience with HCHB
  • Willingness to travel within the assigned coverage area
  • Current BLS certification
  • Valid driver's license.
