The Opportunity

As a Physical Therapist, you'll provide personalized, patient-centered care in the comfort of patients' homes while playing a key role in their rehabilitation journey. Your Day-to-Day
- Perform comprehensive physical assessments and review medical histories to accurately evaluate patient conditions.
- Develop and implement individualized treatment plans aligned with each patient's rehabilitation goals.
- Deliver therapeutic exercises, mobility training, and functional rehabilitation programs.
- Educate patients and family members on recovery expectations, injury prevention, and wellness strategies.
- Monitor patient progress and adjust treatment plans to support optimal outcomes.
- Collaborate with physicians, nurses, and other healthcare professionals to ensure coordinated patient care.
- Maintain accurate, timely patient records and documentation in compliance with regulatory requirements.
- Travel to patient homes throughout the service area to provide care.
What We're Looking For
- Minimum of nine months of experience as a Physical Therapist.
- Previous community or home health experience preferred.
- Master's degree in Physical Therapy from a CAPTE-accredited physical therapist education program.
- Current Physical Therapist license in the state of agency operation and compliance with accepted professional standards and practices.
- Valid CPR certification required and maintained throughout employment in a clinical role.
- Valid driver's license and the ability to travel to all business locations.
