What You’ll Do

• Provide high-quality, patient-centered physical therapy services in patients’ homes.
• Evaluate patients’ mobility, strength, functional abilities, and rehabilitation needs.
• Develop and implement individualized treatment plans focused on improving independence and quality of life.
• Monitor patient progress and adjust treatment plans to achieve the best possible outcomes.
• Educate patients and families on exercises, therapy techniques, safety, and lifestyle modifications.
• Collaborate with nurses, physicians, therapists, and other healthcare professionals to provide coordinated care.
• Maintain accurate and timely documentation of patient visits and progress.

Requirements

• Graduate of an accredited Physical Therapy program.
• Current Connecticut Physical Therapist license.
• At least 1 year of experience as a practicing Physical Therapist.
• Home Health Physical Therapy experience preferred.
• Strong communication and interpersonal skills with a compassionate approach to patient care.
• Ability to work independently, manage priorities, and organize a flexible patient schedule.
• A genuine passion for helping patients achieve greater mobility and independence.
