• Pay Rate: $70.00 / Hour (USD)
• Shift / Schedule: Day (5x8 Hours), 8:00 AM 4:30 PM
• Travel Requirements: Traveler is expected to have their own transportation. Occasional maximum radius of 50 miles. Mileage Reimbursement: IRS Standard.
• Guaranteed Hours: Facility can call off UP TO one shift per pay period (every two weeks)
• Work Authorization: Applicants must be authorized to work for ANY employer in the U.S. We are unable to sponsor or take over sponsorship of an employment Visa at this time.

Job Summary

This is a Home Health Physical Therapist travel/contract opportunity based in Medford, OR and surrounding areas. The role is with a Home Health facility and involves providing skilled physical therapy services to patients in their homes. The system does a good job grouping clients, minimizing drive and windshield time, with an occasional maximum radius of 50 miles.

Key Responsibilities

• Provide skilled home health physical therapy services to patients in their homes within the Medford, OR and surrounding areas.
• May visit up to 6 patients per day; 30 productivity points standard (1:1 ratio).
• Complete Oasis assessments as required (Oasis experience is required please call out if traveler does not have Oasis experience).
• Utilize EPIC/Oasis for charting.

Required Skills

• Oasis documentation experience (required must be called out if absent)
• EPIC/Oasis charting proficiency
• Own reliable transportation

Education Requirements

• PT License required
• BCLS (Basic Cardiac Life Support) certification required

Required Experience

• Minimum 1 year of skilled Home Health experience required
• First-timers not accepted

Work Environment / Physical Requirements

• Home health setting; travel to patient homes throughout Medford, OR and surrounding areas
• Occasional maximum service radius of 50 miles
• Any color scrubs permitted
• Free parking
• Mileage reimbursement at IRS Standard rate
• Facility requires primary series COVID vaccination in addition to one booster
• This facility cannot accommodate 4x10 or 3x12 schedules
• No on-call required
• No weekend requirement
• No floating between units required
• Block schedule not accommodated

• Locals accepted; considered "local" within 50 75 mile radius locals accepted at the same rate.
• Pending license accepted.
• RTO Restrictions: Confirm with hiring manager.
• Traveler is expected to have their own transportation.
• Oasis experience is required. Please call out if traveler does not have Oasis experience.
