Job Title - Physical Therapist-Home Health
Job Type: Contract
Location- Tyler, TX
Shift: Monday-Friday 8:00 am to 4:30 pm
Hours- 40 hrs/ week
Coverage Areas: Hawkins, Mineola, Lindale
Productivity Expectation: 30 points per week
Work Setting: On-site (clinic-based)
Mileage Reimbursement: Yes (between patient visits, from first to last visit)
Pay-$50 to $56/hr on W2
Key Responsibilities:
Key Responsibilities
Perform comprehensive patient assessments in home health and hospice settings
Develop and implement individualized Plans of Care (POC)
Complete required documentation, including:
Start of Care (SOC)
Resumption of Care (ROC)
Recertifications and reassessments
Provide therapeutic interventions to improve patient function and independence
Supervise and collaborate with Physical Therapist Assistants (PTAs)
Educate patients and caregivers on treatment plans and home exercise programs
Ensure compliance with all clinical and regulatory standards
