Physical Therapist (PT) to join our client's growing
Home Health team.
Compensation:Local Pay: $55.00 - $70.00/hour (W-2)
Travel Pay Package: Up to $2,800/week gross (includes weekly non-taxable stipend)*
Location: Warrenton, VA, 20186
Weekly Hour Guarantee: 36
Productivity Expectations (points): 30
Coverage Areas: Fauquier County, Culpeper County, parts of Rappahannock County, parts of Prince William County (all Virginia)
Shift schedule (days, start & end times):
Monday - Friday, 08:00 AM to 04:30 PM
On Call/Weekend Requirement:
None
Will Mileage be reimbursed between visits, from first patient to last patient (Y/N)?
YKey Responsibilities:- Conduct comprehensive home health physical therapy services, including Start of Care (SOC), initial evaluations, routine revisits, re-evaluations, and discharge assessments, while developing individualized treatment plans, improving patients' functional mobility, and meeting a productivity expectation of 30 points.
- Deliver evidence-based therapy interventions, monitor and document patient progress accurately in Homecare Homebase (HCHB)/PointCare, collaborate with physicians and the interdisciplinary team, educate patients and caregivers, and ensure compliance with Medicare, agency, and home health documentation standards across the assigned coverage areas.
Must have qualifications/experience: Licensed Physical Therapist - Virginia PT license; experience with HomeCare HomeBase (HCHB) for documentation and PointCare, Home Health experience.
