What Your Day Might Look Like

• Conduct comprehensive evaluations and develop patient-centered treatment plans
• Collaborate with referring providers, case managers, and fellow rehab team members
• Communicate with patients and families to ensure understanding and continuity of care
• Support rehab diagnoses and adapt care plans as needed

Principal Accountabilities:

• Evaluate outpatients with orthopedic conditions.
• Design and implement PT plan of care.
• Provide evidence-based interventions.
• Document all encounters in the electronic medical record.
• Complies with departmental policies/practices.
• Maintains patient safety, provides patient/family/caregiver education.
• Works cohesively in a team setting.
