Advanced Home Health Care is hiring a full-time Physical Therapist to provide in-home physical therapy services throughout our Southeast Iowa coverage area, including Des Moines, Lee, Henry, Louisa, and Van Buren Counties.

We offer a $100,000 annual baseline salary, negotiable based on experience, flexible scheduling, mileage reimbursement, comprehensive benefits, and the opportunity to play an important role in our growing internal therapy program.

Responsibilities:

• Conduct comprehensive physical therapy evaluations and develop individualized treatment plans
• Provide therapeutic exercises, gait training, balance training, and mobility rehabilitation
• Assess patient progress and adjust treatment plans to support recovery and functional independence
• Educate patients and caregivers on home exercise programs, fall prevention, and safe mobility
• Collaborate with physicians, nurses, and other healthcare professionals to coordinate patient care
• Independently manage patient visits and documentation throughout the assigned service area
• Supervise our PTA staff to ensure treatment plans are followed 

What We're Looking For

Experience Required:

• Minimum 1 year of professional Physical Therapist experience required
• Active Physical Therapist license
• Experience in outpatient, inpatient, rehabilitation, or other clinical settings welcome
• Previous home health care experience preferred, but not required
• Ability to independently manage patient visits, treatment plans, and documentation in a home health setting
