Position Overview


As a Home Health Physical Therapist, you will:

• Deliver individualized physical therapy services in patients’ homes or skilled nursing facilities
  
  
• Evaluate patients and develop customized treatment plans
  
  
• Monitor progress and adjust care plans as needed
  
  
• Collaborate with physicians, caregivers, and interdisciplinary care teams
  
  
• Help patients improve mobility, independence, and quality of life
  
  

Qualifications

• Doctorate or Degree in Physical Therapy (DPT or PT)
  
  
• Minimum 2 years of clinical experience
  
  
• Home Health or Skilled Nursing Facility experience preferred
  
  
• Active California Physical Therapist License
  
  
• Valid California Driver’s License
  
  
• Current CPR certification
  
  
• Current TB test and Professional Liability Insurance
  
  
• Strong communication and organizational skills
  
  
• Ability to work independently and manage a flexible caseload
