Position Summary
•Responsible for establishing, implementing and evaluating a plan of care to restore function and prevent disability following disease, injury or loss of a body part.

Education
•Masters Degree in Physical Therapy or Bachelor’s degree in Physical Therapy with a minimum 5 years of work experience.

Licenses and Certification
•Illinois license in Physical Therapy
•BLS

Experience and Skills
•Technical Experience: N/A

Role Specific Responsibilities
•Performs thorough evaluation
•Develops appropriate Plan of Care
•Implements Plan of Care
•Documentation is accurate, complete and timely
•Provides supervision to assistants, rehab techs, and students
