Description
We are seeking dedicated and compassionate Physical Therapists to join our team. As a Physical Therapist at Family Life care, you will play a vital role in managing the care of our patients by providing compassionate and professional care in the comfort of their own home. FLC is proud to serve our entire community from children to senior citizens.
You will have the opportunity to choose from various assignments, such as patient case manager, long-term care or assisting with recovery from an injury or illness. FLC works with you to build your work schedule around your needs.
The primary duties of a Physical Therapist at FLC are,
Patient assessment, admission, and coordination of care with other disciplines, physicians and care managers
Provide Physical Therapy and treatment of home care patients in accordance with the plan of treatment and with company policies and procedures
Provide Physical Therapy in a pleasant, professional manner to maximize positive outcomes and patient satisfaction
Ensure that all clinical assessments, care plans, starting and resumption of care are completed with agency guidelines. Must be deadline driven
Train, Coach and Direct clinical staff in the performance of their clinical duties.
Responsible for compliance with all state and federal, laws, regulations, policies, and processes relevant to home health
The ideal candidate will have the following experience and approach:
Positive, approachable, and mentoring personality
Understanding of state and federal laws and regulations related to home health, especially Medicare and Medicaid
Experience with Medicare (OASIS), Medicaid and commercial insurance in the home health environment is desired
Strong interpersonal and communication skills, organizational abilities, and attention to detail
Embrace technology and have experience with industry software such as Electronic Medical Records (EMR) and Electronic Visit Verification (EVV) systems.
