Job Summary:
The Physical Therapist delivers high-quality, autonomous physical therapy services either through referral or directly to the consumer. This role evaluates patients, develops personalized treatment plans, monitors progress, and adapts care to achieve optimal patient outcomes. The therapist also contributes to program development and enhancement to meet patient and community needs while ensuring compliance with quality, safety, and patient experience standards.

Key Responsibilities:
  • Perform evaluations and design individualized treatment programs to achieve patient-specific goals.
  • Assess patient responses to therapy and modify treatment plans as appropriate.
  • Maintain accurate and compliant patient documentation according to policies and procedures.
  • Meet productivity expectations, including access to care, patient experience, and quality metrics.
  • Assist in the development and improvement of programs to meet evolving community and patient needs.
  • Adapt care delivery to accommodate patient-specific factors such as privacy, communication style, and cultural considerations.
  • Perform additional related duties as assigned.
  • Stay current on applicable federal, state, and local laws, as well as accreditation and regulatory requirements.
  • Immediately report any compliance, safety, or privacy concerns.

Required Experience:
  • Bachelor’s degree in Physical Therapy.
  • Minimum of 2 years of physical therapy experience.
  • Current physical therapy license in the state of practice.

Nice-to-Have Experience:
  • Experience in program development and community outreach.
  • Prior work in high-volume patient care environments.

Required Skills:
  • Patient evaluation and treatment planning.
  • Clinical documentation and compliance.
  • Effective verbal and written communication.

Preferred Skills:
  • Computer literacy and ability to use software and web-based applications.
  • Strong interpersonal and relationship-building abilities.
  • Skilled in judgment and decision-making in complex patient care situations.

Additional Skills:
  • Flexibility in scheduling based on patient demand.
  • Adherence to infection control and safety protocols.
  • Ability to work in environments with exposure to communicable diseases and hazardous materials.
