Job Summary
The role is responsible for coordinating and providing restorative and rehabilitative Physical Therapy services to participants at the Center and at home. This position involves close coordination with physicians, rehabilitation staff, and other interdisciplinary team (IDT) members to maximize participant independence and safety, as well as to develop or restore function. The role collaborates with Occupational Therapists to efficiently operate the rehabilitation department and facilitates collaborative relationships with internal and external stakeholders to promote organizational values, team culture, and mission.
Duties and Responsibilities
- Conduct assessments to determine care needs, preferences, and goals of the participants; actively participate in IDT meetings to develop participant care plans.
- Deliver and document interventions in accordance with physician orders and/or as agreed upon in the participants' care plans, including active and passive exercise, muscle re-education, gait and functional training of ADLs, and treatments such as massage, heat, sound, water, light, or electricity.
- Supervise Therapy Assistants, providing training, instruction, and guidance needed to implement Functional Maintenance Programs (FMP).
- Provide training and instructions to staff, participants, and caregivers/families in the use and care of therapeutic equipment.
- Order, track, and manage maintenance for equipment.
- Remain alert to pertinent input from team members, participants, and caregivers; promptly update the IDT of any changes in participants' condition or functional status.
- Work with other IDT members to support smooth care transitions between settings (e.g., hospitals, skilled nursing facilities, home, etc.).
- Support enrollment of prospective participants into the program in coordination with the Marketing Team.
- Participate in end-of-life care coordination and support.
- Manage direct reports, including training, supervising, and mentoring.
- Be legally authorized to practice in position location; only act within the scope of authority to practice.
- Complete all required documentation in a timely and accurate manner.
- Protect privacy and maintain confidentiality of all company procedures and information about team members, participants, and families.
- Follow organizational policies and procedures; participate in required Quality Improvement activities, staff training, and meetings.
- Communicate regularly with Supervisor and team regarding workload and priorities.
- Complete all mandated trainings and education in a timely manner.
- Complete all mandated occupational health screenings as needed.
- Exercise flexibility in performing assignments as business needs evolve.
- Other duties as assigned.
Skills
Required Skills & Experience:
- Five years of progressively responsible experience in the field.
- Minimum of two years' experience in a clinical setting with a frail or elderly population.
- Knowledge and skills necessary to treat frail, elderly participants with complex needs.
- Highly motivated and self-directed; able to execute tasks in a quickly changing environment and make sound decisions in emergency situations.
- Two years' supervisory experience with demonstrated ability to mentor and develop team members.
- Ability to process accurate information in a timely manner.
- Excellent leadership, organizational, and communication skills in settings with seniors, their families, and interdisciplinary team members.
- Experience leading in a data-driven organization, leveraging reports and data to prioritize and manage people and projects.
- Highly motivated, inspirational leader who can innovatively engage, support, and remove barriers for the team to ensure consistent goal achievement.
- Ability to work independently with minimal supervision.
- Demonstrated ability to prioritize in a fast-paced environment.
- Experience and competency working with people from diverse backgrounds and cultures.
