Job Title: Physical Therapist

Salary Range: $40-$43 per hour

Hours: 30-40 hours per week, Monday through Friday, no weekend rotation

Facility: 400-bed facility, including a Long-Term Care unit with 40 beds, an Assisted Living unit with 100 beds, and an Independent Living Facility with 250-275 beds. The facility also has two licensed buildings, one with an Assisted Living unit and another with a combination of Assisted Living and Long-Term Care units. There is also an Assisted Living Dementia unit.

Job Summary:

We are seeking a highly skilled and dedicated Physical Therapist to join our team at our 400-bed facility. The ideal candidate will have a passion for helping patients improve their physical function and overall well-being. As a Physical Therapist, you will work closely with patients of all ages and conditions to develop and implement individualized treatment plans.

Key Responsibilities:

- Conduct initial evaluations and assessments to determine patients' physical therapy needs

- Develop and implement treatment plans based on patients' goals and needs

- Utilize a variety of techniques and modalities to help patients improve their physical function and reduce pain

- Monitor and document patients' progress and adjust treatment plans as necessary

- Educate patients and their families on exercises and techniques to continue their progress outside of therapy sessions

- Collaborate with other healthcare professionals to ensure comprehensive and coordinated care for patients

- Stay current with advancements and developments in the field of physical therapy

Qualifications:

- Bachelor's, Master's, or Doctorate degree in Physical Therapy

- Valid state license to practice as a Physical Therapist

- Minimum of 2 years of experience in a healthcare setting, preferably in a skilled nursing facility or assisted living facility

- Strong knowledge of physical therapy principles and techniques

- Excellent communication and interpersonal skills

- Ability to work independently and as part of a multidisciplinary team

- Compassionate and patient-focused attitude

- CPR certification
