Full-Time | Lee County, FL (Including Bonita Springs & Estero)
A growing home health organization is seeking a compassionate and dedicated Physical Therapist (PT) to join its team in Lee County, Florida. This full-time opportunity allows you to provide one-on-one care directly in patients' homes while making a meaningful impact on their recovery, mobility, and overall quality of life.
This role is ideal for a Physical Therapist who values patient-centered care, autonomy, flexibility, and the ability to build strong relationships with patients and families in a home health setting.
Responsibilities
Perform initial and ongoing patient evaluations and reassessments, including OASIS documentation
Develop and implement individualized treatment plans in accordance with physician orders
Provide skilled physical therapy services in patients' homes
Monitor patient progress and adjust treatment plans as needed
Communicate patient status updates and treatment responses to physicians and interdisciplinary team members
Educate patients and family members on exercises, mobility, safety, and use of assistive devices
Assist with discharge planning and completion of discharge summaries
Supervise Physical Therapy Assistants (PTAs) and Home Health Aides in accordance with agency and state guidelines
Complete accurate and timely clinical documentation and progress notes
Participate in case conferences, staff meetings, and interdisciplinary care planning
Coordinate outpatient services or additional care needs when appropriate
Ensure compliance with all Medicare, state, and agency regulations
Qualifications
Graduate of an accredited Physical Therapy program approved by CAPTE or equivalent
Active Florida Physical Therapist license
Minimum of 1 year of institutional or home health experience preferred
Current CPR certification (online-only certifications not accepted)
Strong communication and clinical assessment skills
Ability to work independently and manage a mobile patient caseload
Valid driver's license, reliable transportation, and auto insurance required
Must successfully pass background screening
Physical Requirements
Ability to travel throughout Lee County, including Bonita Springs and Estero
Ability to access patient homes that may not be wheelchair accessible
Ability to lift, move, and assist patients as needed
Must be able to bend, stoop, reach, and perform physical patient care duties
Strong verbal communication skills in English required
