Responsibilities

• Assess patient needs and develop targeted treatment plans.

• Deliver evidence-based physical therapy interventions for optimal recovery.

• Document sessions and track patient progress in the EMR system.

• Collaborate with multidisciplinary teams to coordinate care.

• Set and monitor personal performance metrics and patient outcomes.

• Identify patients suitable for referrals and participate in pipeline growth.

• Demonstrate accountability for own outcomes within a results-driven environment.

• Educate patients and families on exercises and self-management strategies.

• Maintain adherence to safety, hygiene, and clinic protocols.

• Engage in ongoing professional development and attend relevant trainings.

• Provide timely progress updates to supervisors and document milestones.

• Support clinic-wide initiatives to improve patient access and throughput.

Experience/Qualifications

• Licensed Physical Therapist with active state license

• Assess patients and design targeted, evidence-based treatment plans

• Deliver PT interventions and document progress in EMR accurately

• Demonstrate accountability for outcomes in a results-driven environment

• Collaborate with multidisciplinary teams to coordinate care
