Position Overview
McLeod Health is seeking a motivated Physical Therapist to join our Outpatient Rehab/Sports Medicine team in Hartsville, SC. This role focuses on delivering high‑quality, patient‑centered care to individuals with musculoskeletal, neurological, cardiovascular, and developmental conditions. You will evaluate, plan, and implement evidence‑based treatment programs designed to reduce pain, restore function, and promote long‑term health and wellness.
Responsibilities
- Conduct comprehensive evaluations using patient history, observation, and objective measures; develop individualized plans of care tailored to patient learning styles and therapeutic needs.
- Provide therapeutic interventions for a wide range of physical and functional limitations, documenting progress with measurable, functional goals.
- Maintain accurate and timely documentation including evaluations, re‑evaluations, treatment notes, charges, and discharge summaries according to departmental guidelines.
- Support departmental operations by assisting with schedule adjustments, supervising treatment teams, and collaborating regularly with PTAs to review patient goals and progress.
- Contribute to departmental growth through professional development activities such as competency training, serving as a clinical instructor, mentoring, program leadership, and performance improvement initiatives.
- Demonstrate ongoing professional growth by setting goals, participating in continuing education, and maintaining CEUs required by SC LLR.
- Communicate effectively with physicians, interdisciplinary team members, and other medical personnel regarding patient progress, treatment recommendations, and order clarification.
- Ensure a safe and organized work environment by using appropriate health and safety equipment and maintaining clean, orderly treatment areas.
- Perform other duties as assigned.
Work Schedule:
- Full-Time
- 72-80 hours biweekly

Education & Requirements
- Active South Carolina Physical Therapy License
- Current Basic Life Support (CPR/AED) Certification
