Medford, OR. This is an excellent opportunity for a licensed therapist who enjoys helping residents regain mobility and independence while working a consistent weekday schedule.
Pay Details:$52.86 - $55.44

Position Details:Location: Medford, OR
Schedule: Monday through Friday
Shift: Variable Shift
Hours: 40 Hours per Week

Certification Requirements:- Physical Therapist License - Oregon (Required)
- BLS Certification (Required)
Skill Requirements:- Licensed to practice as a Physical Therapist in Oregon
- Ability to develop and implement individualized treatment plans
- Strong clinical assessment and documentation skills
- Effective problem-solving and decision-making abilities
- Excellent communication and interpersonal skills
- Prior experience in long-term care, rehab, or post-acute settings preferred
Key Responsibilities:- Evaluate residents to determine therapeutic needs and potential
- Create, implement, and modify treatment plans as necessary
- Provide direct therapy services in accordance with physician-approved plans
- Document evaluations, treatments, progress notes, and charges accurately
- Participate in discharge planning to support ongoing patient success
- Supervise Physical Therapist Assistants and Aides
- Screen residents for potential benefit from physical therapy interventions
- Ensure compliance with facility, state, and federal standards
