Description

Pride Health is hiring a Physical Therapist to support our client's medical patients in Nutley, NJ (Coverage within Newark, Nutley, Belleville, and Bloomfield in New Jersey). This is a 6-month assignment (Possibility of extension or conversion) and a great way to start working with a top-tier healthcare organization!

Location: Nutley, NJ 07110 with Travel within Newark, Nutley, Belleville, and Bloomfield in New Jersey

Job Type: Local Contract

Duration: 6+ months Contract (Possibilities of extension)

Pay rate: $50 - 56/hr. On W2 (Negotiable - Pay Rate is based on experience)

Schedule: Mon - Fri (5 days/ wk.) from 8 AM to 4:30 PM (30-hour weekly guarantee)

Mileage: Reimbursement provided (from second patient to last patient)

Job Responsibilities:

  • Perform Start of Care (SOC) visits, completing initial assessments and establishing individualized Plans of Care (POC) in accordance with physician orders.
  • Conduct thorough physical therapy evaluations assessing strength, mobility, balance, pain, and functional limitations.
  • Provide skilled therapy interventions during routine and follow-up (revisit) appointments.
  • Perform re-evaluations to assess patient progress, update treatment plans, and adjust goals as needed.
  • Develop and implement evidence-based treatment plans tailored to each patient's needs and goals.
  • Educate patients and caregivers on home exercise programs, safety precautions, and disease management.
  • Complete timely and accurate clinical documentation in compliance with regulatory and organizational standards.
  • Coordinate care with physicians, interdisciplinary team members, and caregivers to ensure optimal patient outcomes.
  • Conduct discharge visits, including final assessments, patient education, and transition planning.

Qualifications:

  • Graduate of an accredited Physical Therapy program (bachelor's or higher).
  • Active, unrestricted Physical Therapist (PT) license in New Jersey.
  • Minimum 1 year of clinical experience (home health experience preferred).
  • Valid driver's license and reliable transportation.
  • Proficiency with Homecare Homebase/ PointCare (preferred).
  • Strong working knowledge and experience with OASIS documentation.
