About the RoleWe are seeking an experienced
Physical Therapist for a 13-week Home Health contract in
Contra Costa County, California. In this role, you will provide individualized therapy services in patients' homes, helping improve mobility, independence, and overall quality of life. The ideal candidate is organized, patient-focused, and experienced in managing a home health caseload while maintaining accurate clinical documentation.
Responsibilities- Perform comprehensive patient evaluations and develop individualized treatment plans to support functional recovery.
- Deliver skilled physical therapy services in the home health setting while prioritizing start-of-care visits.
- Supervise Physical Therapy Assistants (PTAs) and collaborate with the interdisciplinary care team.
- Complete clinical documentation, including start-of-care reports and treatment notes, within required timeframes.
- Maintain ongoing communication with clinical supervisors and office staff to ensure coordinated patient care.
Requirements- Active California Physical Therapist (PT) License required.
- Minimum 1 year of Home Health Physical Therapy experience required.
- Ability to travel throughout Contra Costa County and surrounding areas as needed.
- Strong documentation, communication, and patient care coordination skills.
- Ability to manage productivity expectations while delivering high-quality patient care.
