Shift: Monday-Friday, with required availability between 7:00 AM and 6:00 PM

Responsibilities:
  • Perform Physical Therapy evaluation and development of the plan of care appropriate for each patient based on the patient’s physical, mental, emotional, and developmental impairments and functional limitations
  • Participate in quality improvement initiatives and stay current with best practices in physical therapy

  • Provide Physical Therapy interventions in accordance with established standards of care, policies, procedures, and state/federal law
  • Supervise Physical Therapist Assistant and Rehab Aides
  • Complete appropriate documentation for Physical Therapy and Physical Therapy Assistant students as assigned by CCCE
  • Monitor patient progress and adjust treatment plans as necessary

  • Participate in patient care conferences/ staffing as appropriate for multidisciplinary patients
Qualifications:

Education: 

  • Bachelor’s degree from an accredited Physical Therapy program prior to 2005
  • Master’s degree from an accredited Physical Therapy program between 2005-2019
  • Clinical Doctorate from an accredited Physical Therapy program 2020-Present

License(s)/Certification(s): 

  • Current IA and IL Physical Therapy licensure (or eligible) upon hire
  • Current CPR certification.
