Key Responsibilities

Develop and update individualized care plans

Provide physical therapy services per physician's plan of care

Assess functional status, mobility, and home environment

Educate patients and families on therapy techniques and equipment

Maintain accurate documentation and communicate progress to physicians

Requirements

Licensed Physical Therapist in the State of Nevada

Degree from a CAPTE-accredited program; passed state-approved exam

Minimum 2 years' experience (home health preferred)

Current CPR certification

Valid driver's license, insurance, and reliable transportation
