The part-time (29 hours/week) Physical Therapist serves as a member of a multi-disciplinary team to provide individual and group services to children enrolled in the DDI Vantage Early Intervention Program.
Responsibilities:
Conducts developmental evaluations using appropriate assessment tools to assist in determining program eligibility and helps complete Individualized Family Service Plans (IFSP).
Administers, interprets, and evaluates tests given to infants and toddlers enrolled in DDI Vantage and uses criterion- and norm-referenced instruments to evaluate ongoing program and progress.
Assists parents and caregivers with the identification of motor outcomes for IFSPs.
Provides direct individual and group intervention and treatment to enhance the child's motor development and facilitates parents and caregivers in implementing a home program to achieve the desired outcomes.
Facilitates physical activities, recommends assistive devices and durable medical equipment when appropriate, and educates parents, caregivers, and staff on their use.
Provides written reports on a child's progress to third parties when requested.
Assists parents and caregivers in their role as a child advocate when appropriate.
Identifies social service needs for families when appropriate and refers them to the program coordinator for help in accessing community resources.
Collaborates with related disciplines to ensure appropriate service delivery for each child.
Provides motor consultation, education, and other advisory services to parents, caregivers and staff to assist in home- and center-based intervention.
Possesses understanding and knowledge of a child's overall development and seeks consultation from other disciplines when appropriate.
Assists in child-find activities.
Attends all mandatory CSPD training for early intervention credentialing under Part C rules.
Participates in staff in-service training, serves on committees, attends workshops and conferences as requested or approved, and may provide in-service training to staff.
Attends team meetings as necessary and all mandatory DDI Vantage meetings.
Demonstrates professionalism in all aspects of the job.
Safeguards the confidentiality of all clients, at all times.
Completes written documentation in a timely manner to ensure program compliance.
Fulfills other duties as assigned.
