Physical Therapist

Pride Health is hiring a Travel Physical Therapist to support our client’s home health services in Pensacola, FL (32504). This is a 3+ month contract assignment and a great opportunity to work with a leading healthcare organization while gaining valuable home health experience.

Job Summary:
The Travel Physical Therapist will provide high-quality, in-home patient care focused on rehabilitation, mobility improvement, and restoring functional independence, while maintaining strong documentation and productivity standards.

Key Responsibilities:

• Provide physical therapy services to patients in their homes
• Complete a minimum of 6 patient visits (points) per day
• Conduct patient evaluations and develop individualized treatment plans
• Deliver therapeutic exercises and rehabilitation programs
• Document patient progress using OASIS and maintain accurate records
• Educate patients and caregivers on treatment plans and recovery goals
• Travel daily to patient homes within the Pensacola to Destin coverage area
• Ensure compliance with healthcare regulations and safety standards

Required Qualifications:

• Associate’s Degree or higher in Physical Therapy
• Active Physical Therapist license (Florida preferred)
• OASIS proficiency required
• Home health experience preferred
• Strong clinical, documentation, and time management skills
• Ability to work independently in a field-based setting

Additional Information:

• Shift: Monday – Friday, 8:00 AM – 4:30 PM
• Location: Pensacola, FL 32504 (coverage: Gulf Breeze to Destin area)
• Duration: 3+ Month Contract
• Travel: Daily travel to patient homes required
• Dress Code: Royal blue scrubs or business casual (dockers with polo)
