Physical Therapist - Per Diem

We are seeking a dedicated and compassionate Physical Therapist to join our homecare team in a per diem capacity. This role involves providing personalized physical therapy services to patients in their homes, supporting their recovery and improving their quality of life. The position offers flexibility with an expected caseload of between 6 to 15 visits per week, allowing for a balanced work schedule. Responsibilities

• Deliver high-quality, individualized physical therapy services to patients in their homes, following physician prescriptions and treatment plans.

• Assess patients’ physical conditions, develop tailored therapy plans, and adjust interventions as needed to promote optimal recovery.

• Educate patients and their families on exercises, mobility techniques, and injury prevention strategies to support ongoing health and independence.

• Maintain accurate and detailed documentation of patient assessments, treatment sessions, progress notes, and discharge summaries in compliance with regulatory standards.

• Coordinate with healthcare providers, caregivers, and family members to ensure cohesive patient care and communication.

• Manage a flexible schedule with 6 to 15 patient visits per week, ensuring timely and efficient service delivery.

Requirements

• Valid state licensure as a Physical Therapist.

• Proven experience or strong interest in homecare physical therapy.

• Excellent interpersonal skills with the ability to connect with diverse patient populations.

• Ability to work independently with strong organizational skills and self-motivation.

• Reliable transportation and the flexibility to accommodate a variable schedule of patient visits.
