Job Description Physical Therapist (Home Health Travel Assignment | Smyrna Health & Wellness Center, Smyrna, DE)

Assignment Details

  • Facility: Smyrna Health & Wellness Center Smyrna, DE
  • Position: Physical Therapist (Travel)
  • Duration: 13 Weeks
  • Schedule: MondayFriday, 8-hour day shifts (40 hours/week)
  • Guaranteed Weekly Hours: 40
  • Pay Cycle: Weekly Pay
  • On-Call/Weekends: To be discussed with the facility
  • Local Radius: 50-mile radius applies for local pay rate
  • Credentialing: Allow approximately 5 weeks for credentialing. Non-Delaware licensed candidates must complete in-person fingerprinting in Delaware after acceptance.
  • Fingerprint results typically take 34 weeks to process.
  • Required Experience: Home Health experience with proficiency in OASIS documentation.
  • Dress Code: Teal scrubs.
  • Expected Start Date: August 17.
  • Provide skilled physical therapy services to adult and geriatric patients in a home health setting throughout Sussex County, Delaware.
  • Perform comprehensive initial evaluations, reassessments, OASIS assessments, and develop individualized treatment plans based on physician orders and patient goals.
  • Deliver evidence-based interventions including therapeutic exercises, gait training, balance training, mobility training, strengthening, transfer training, and fall prevention.
  • Educate patients and caregivers on home exercise programs, assistive devices, pain management, disease processes, and strategies to maximize functional independence.
  • Maintain accurate and timely OASIS and daily clinical documentation while meeting productivity expectations within an 8-hour workday.
  • Manage a daily caseload consisting of approximately 6 revisits (typically including one evaluation) or 45 revisits and one admission.
  • Collaborate with physicians, nurses, occupational therapists, speech therapists, social workers, and case managers to coordinate comprehensive patient-centered care.
  • Assess patient progress, modify treatment plans as appropriate, and communicate changes in patient condition to the interdisciplinary healthcare team.
  • Ensure compliance with Medicare regulations, agency policies, infection prevention standards, and home health documentation requirements.
  • Travel independently to patient homes within the assigned territory, with occasional coverage outside the primary region as operational needs require.
  • Primarily provide care within one of the following service regions:
    • Georgetown Region: Georgetown, Harbeson, Ellendale, and Millsboro.
    • Lewes/Rehoboth Beach Region: Lewes, North Shores, and Rehoboth Beach.
  • Maintain a high level of patient safety, quality care, and customer service while working autonomously in the community.
  • Participate in interdisciplinary case conferences and discharge planning to promote optimal patient outcomes.
