Key Responsibilities:

Assess patients and develop individualized treatment plans to improve movement and function
Provide hands-on therapy and exercises to enhance strength, mobility, and balance
Educate patients and caregivers on rehabilitation techniques and injury prevention
Document patient progress and collaborate with interdisciplinary healthcare teams
Ensure compliance with facility policies and state/federal regulations

Qualifications:

Active Physical Therapist (PT) license in CA
Experience in skilled nursing, rehabilitation, or long-term care preferred
Strong communication, patient-care, and teamwork skills
Passion for helping patients achieve their highest level of independence
