Position Summary


St. Jude Home Health Agency is seeking a compassionate and skilled Physical Therapist to join our dedicated home health team. You’ll provide rehabilitative services to patients in their homes, helping them regain mobility, independence, and quality of life.
Key Responsibilities

• Conduct in-home physical therapy evaluations and develop individualized care plans
• Implement therapeutic exercises and interventions based on patient needs
• Collaborate with interdisciplinary team members including nurses, occupational therapists, and physicians
• Educate patients and caregivers on exercises, safety, and recovery strategies
• Document patient progress and outcomes in compliance with Medicare and agency standards

Qualifications

• Current California Physical Therapist license
• Minimum of 1 year experience in home health or geriatric care preferred
• CPR certification
• Strong communication and time management skills
• Familiarity with EMR systems and Medicare documentation standards
