Physical Therapist – PRN/On-Call | New Grads Welcome!

Make a meaningful impact with a flexible PRN role that currently offers approximately 14 regularly scheduled hours per week. Join our supportive rehab team in a welcoming facility with modern therapy spaces, where you’ll help residents regain independence and return home safely after illness or surgery.

In this role, you’ll provide physical therapy and supervise one of our two Physical Therapist Assistants. Whether you’re an experienced PT seeking flexible hours or a new grad looking for mentorship, we’ll give you the support and tools to succeed.

Key Responsibilities

• Evaluate residents and develop personalized treatment plans to address functional deficits, mobility needs, and rehabilitation goals.
• Provide direct therapy services in accordance with physician orders and industry standards.
• Collaborate with an interdisciplinary team—including nurses, physicians, and other therapists—to ensure holistic resident care.
• Educate residents, families, and caregivers on therapeutic techniques and strategies to promote independence and safe mobility.
• Accurately document evaluations, progress notes, and treatment outcomes in compliance with regulatory and facility requirements.

Qualifications

• Current Physical Therapist license in the state of Nebraska (or eligibility).
• Degree in Physical Therapy from an accredited program.
• Strong clinical and communication skills, with the ability to motivate and encourage residents.
• Previous experience in a skilled nursing, assisted living, or rehabilitation setting preferred.
