LeeKO Home Health LLC is looking for a passionate and dedicated PRN Physical Therapist to join our growing team in Orlando, FL. Responsibilities:

• Conduct comprehensive physical therapy evaluations and assessments in patients' homes
• Develop and implement individualized treatment plans to restore function and mobility
• Provide therapeutic interventions including therapeutic exercise, manual therapy, and neuromuscular re-education
• Monitor and document patient progress, adjusting care plans as needed
• Educate patients and caregivers on home exercise programs and injury prevention
• Collaborate with physicians, nurses, and other healthcare team members to ensure coordinated care
• Maintain accurate and timely clinical documentation in compliance with home health regulations

Requirements:

• Valid Florida Physical Therapist (PT) license in good standing
• Graduate of an accredited Physical Therapy program (DPT or equivalent)
• Home health experience preferred but not required
• Current CPR/BLS certification
• Reliable transportation and valid driver's license for home visits throughout the Orlando area
• Strong clinical assessment, critical thinking, and communication skills
• Ability to work independently and manage a flexible PRN schedule
