ESSENTIAL
FUNCTIONS:
Conduct
comprehensive assessments to individuals by assessing their medical history,
motion of movements, and evaluating physical limitations to diagnose movement
dysfunctions.
Develop
the appropriate therapy treatment interventions based on the needs of the
individual, in addition report progress of long- and -short term treatment
goals.
Evaluates
current mobility, strength and diagnosis of individuals to determine if therapy
treatment interventions need to be modified.
Assist
individuals with therapy by administering manual exercises, giving massages and
using therapeutic equipment as necessary.
Provides
support, counsel, advice, training and education for individuals, family,
caregivers and other professionals on the importance of being consistent with
therapy treatment plans.
Efficiently
assembles extensive documentation on individuals’ progress, care services and
therapy adjustments, when necessary, that aligns with standard medical
documentation and facility protocol/procedures documentation.
Collaboratively
work with other healthcare professionals to provide comprehensive care.
Must
display excellent active listening skills, critical thinking skills,
verbal/written communication skills, time management skills.
Must
display compassion, integrity and respect to all individuals.
Supporting
additional responsibilities and/or tasks as needed for the department.
PHYSICAL REQUIREMENTS:
Physically,
be able to lift, transfer and assist patients while providing therapy
interventions during physical activity.
Must be
able to lift and carry items up to 25lbs, including assisting individuals when
needed.
Ability
to visually be able to perform job duties, including examining individuals and
reading information from printed sources and computer screens.
OTHER REQUIREMENTS:
Education: Must be a graduate of an accredited Physical Therapy program.
Licenses/Certifications: Must hold an active and in good standing Wisconsin Physical Therapist License and CPR Certification.
Follows/adheres to organization, federal, and state regulations and guidelines, including Resident/Client rights
Maintains neat, well-groomed appearance, adhering to dress code of organization/program
