• Productivity Target: Averages around 83 - 85% but full details to be discussed during the interview with DOR
• Team: You'll partner with 2 PTs, 4 PTAs, 3 OTs, 2 COTAs and 3 SLPs

• New Grads Welcome: Pending California licensure considered, with structured mentorship from our Director of Rehabilitation

You'll see a wider range of patients here than at most skilled nursing buildings. Our skilled nursing residents are working to regain strength and mobility after hospital stays, while our subacute unit serves medically complex patients, including [ventilator and tracheostomy patients], where early mobility makes a measurable difference. You'll work closely with respiratory therapists and nursing, with [mentorship for clinicians new to subacute care]. Your evaluations set the direction, and your team helps carry it out.

What You'll Do:

• Complete PT evaluations and reevaluations, and build function based plans of care
• Treat residents individually and in groups to restore mobility, strength, and independence
• Supervise PTAs and guide rehab aides and clinical students
• Train residents and families on home exercise programs and safe use of assistive devices
• Contribute to care conferences, MDS input, and discharge planning [with protected time for documentation and meetings]
• Complete timely, accurate documentation

What You Need:

• California PT license, or pending licensure
• Graduate of an accredited PT program and passed the NPTE
• Current BLS/CPR, or willingness to obtain
• Skilled nursing, subacute, or hospital experience preferred but not required
• Ability to safely assist with transfers, mobility, and repositioning
