We are seeking a licensed Physical Therapist (PT) to join our home health team. The PT will provide individualized rehabilitation programs to help patients regain mobility, reduce pain, and improve overall physical function in a home care setting.

Key Responsibilities

• Perform comprehensive physical assessments of patients
• Develop and implement individualized treatment plans
• Provide therapeutic exercises and mobility training
• Educate patients and families on recovery and prevention strategies
• Monitor patient progress and adjust care plans accordingly
• Maintain accurate and timely documentation
• Collaborate with interdisciplinary healthcare teams

Requirements

• Degree in Physical Therapy from an accredited program
• Active Ohio PT license
• Strong clinical and assessment skills
• Home health or post-acute experience preferred
• New graduates welcome

Work Details

• Setting: Home Health
• Patient Type: Geriatric / Post-acute
• Schedule: Monday-Friday, 9:00 AM - 5:30 PM
• Patient Load: 4-6 patients/day
• Travel: 25-30 mile radius (mileage reimbursed)

Compensation & Benefits

• Pay: $42-$58+ per hour
• Mileage reimbursement: $0.40/mile
• PTO, 401(k), pension, sick leave (eligibility applies)
