PHYSICAL THERAPIST (PT) – LYMPHEDEMA / PELVIC FLOOR (PERMANENT, SNF)

Location: CA 91316
Start Date: ASAP
Schedule: Permanent (1 Position – Lymphedema or Pelvic Floor)

Compensation:

• $50 – $58 per hour

Position Summary & Responsibilities (SNF)

• Evaluate and treat SNF residents in Lymphedema or Pelvic Floor specialty

• Provide skilled therapy services and documentation

• Ensure Medicare/PDPM compliance

• Collaborate with interdisciplinary care team

• Maintain productivity and quality standards

Requirements

• Active Physical Therapist (PT) license

• SNF or post-acute rehab experience required

• Lymphedema or Pelvic Floor experience preferred

• Knowledge of Medicare/PDPM and documentation standards

• Strong communication and teamwork skills
