About the Role: Star Light Home Health is looking for a passionate and dedicated Physical Therapist (PT) to join our growing Medicare Certified Home Health team. Responsibilities:
Conduct comprehensive physical therapy evaluations and develop individualized treatment plans for home health patients
Deliver skilled PT services including therapeutic exercise, gait training, balance and fall prevention, and pain management
Monitor and document patient progress in compliance with Medicare home health guidelines and agency policies
Collaborate with the interdisciplinary care team including nurses, occupational therapists, and physicians
Educate patients and caregivers on home exercise programs, safety techniques, and functional independence strategies
Complete OASIS assessments and maintain accurate, timely clinical documentation
Participate in care coordination and discharge planning to ensure continuity of care
Requirements:
Active Colorado Physical Therapist (PT) license in good standing
Graduate of an accredited Physical Therapy program (DPT or equivalent)
Experience in home health, geriatrics, or Medicare-certified settings preferred
Familiarity with OASIS documentation and Medicare home health regulations
Valid driver's license, reliable transportation, and ability to travel to patient homes in the Grand Junction area
Strong communication, time management, and critical thinking skills
Compassionate, self-motivated, and comfortable working independently in the field
