Location: Loveland, CO
Coverage Area: Boulder, Longmont, Erie, Superior, Keenesburg, Brighton, Fort Lupton, Firestone
Employment Type: Full-Time
Schedule: Monday – Friday | 8:00 AM – 5:00 PM
Compensation: $101,000 – $112,000 annually
Additional Compensation: Sign-On Bonus Available
Work Environment: Field-Based Home Health

Position Overview

Wheeler Staffing Partners is seeking a compassionate and patient-focused Physical Therapist (PT) for a full-time Home Health opportunity based in Loveland, Colorado. This role is responsible for providing individualized physical therapy services to patients within their homes while supporting rehabilitation, mobility improvement, pain management, and overall functional independence.

The ideal candidate will have strong clinical assessment skills, excellent communication abilities, and a passion for delivering high-quality patient care in a home health environment.

Key Responsibilities

• 
  Provide skilled physical therapy services to patients in the home health setting
  
• 
  Evaluate patient conditions, mobility limitations, strength deficits, and rehabilitation needs
  
• 
  Develop individualized treatment plans based on physician orders and patient assessments
  
• 
  Implement therapeutic interventions to improve mobility, balance, strength, endurance, and functional independence
  
• 
  Educate patients and caregivers on therapy exercises, treatment plans, safety precautions, and recovery strategies
  
• 
  Collaborate with physicians, nurses, case managers, and interdisciplinary healthcare teams to coordinate patient care
  
• 
  Monitor patient progress and adjust treatment plans as appropriate
  
• 
  Maintain accurate, timely, and compliant documentation of patient visits, care plans, and progress notes
  
• 
  Participate in on-call rotation as required
  

Required Qualifications

Education

• 
  Master’s degree in Physical Therapy from a CAPTE-accredited program required
  

Licensure & Certifications

• 
  Active Physical Therapist (PT) license in the state of Colorado required OR willingness to obtain Colorado licensure if currently licensed in another state
  
• 
  Current CPR certification required
  
• 
  Valid driver’s license and reliable transportation required
  

Experience

• 
  Minimum of 9 months of experience as a Physical Therapist required
  
• 
  Home health or community-based therapy experience preferred

  Knowledge of rehabilitation techniques, mobility training, and therapeutic exercise programs
