Position OverviewWheeler Staffing Partners is seeking a compassionate and patient-focused Physical Therapist (PT) for a full-time Home Health opportunity based in Loveland, Colorado. This role is responsible for providing individualized physical therapy services to patients within their homes while supporting rehabilitation, mobility improvement, pain management, and overall functional independence.
The ideal candidate will have strong clinical assessment skills, excellent communication abilities, and a passion for delivering high-quality patient care in a home health environment.
Key Responsibilities
Provide skilled physical therapy services to patients in the home health setting
Evaluate patient conditions, mobility limitations, strength deficits, and rehabilitation needs
Develop individualized treatment plans based on physician orders and patient assessments
Implement therapeutic interventions to improve mobility, balance, strength, endurance, and functional independence
Educate patients and caregivers on therapy exercises, treatment plans, safety precautions, and recovery strategies
Collaborate with physicians, nurses, case managers, and interdisciplinary healthcare teams to coordinate patient care
Monitor patient progress and adjust treatment plans as appropriate
Maintain accurate, timely, and compliant documentation of patient visits, care plans, and progress notes
Participate in on-call rotation as required
Required QualificationsEducation
Masters degree in Physical Therapy from a CAPTE-accredited program required
Licensure & Certifications
Active Physical Therapist (PT) license in the state of Colorado required OR willingness to obtain Colorado licensure if currently licensed in another state
Current CPR certification required
Valid drivers license and reliable transportation required
Experience
Minimum of 9 months of experience as a Physical Therapist required
Home health or community-based therapy experience preferred
Required Skills & Competencies
Strong patient assessment and clinical decision-making skills
Knowledge of rehabilitation techniques, mobility training, and therapeutic exercise programs
Excellent communication and patient education abilities
Ability to work independently in a field-based environment
Strong organizational and documentation skills
Ability to collaborate effectively within an interdisciplinary care team
