Enrich the lives of older adults through community, choice, and continuing care in the Physical Therapist (PT) role. In this unique and rewarding role, you will help patients improve mobility, manage pain, and maintain independence—while also supporting comfort and quality of life for those in hospice. The Physical Therapist is solely responsible for evaluating and diagnosing a patient, preparing a treatment plan and therapeutic goals, including determining number and frequency of visits providing or supervising all care, and determining appropriate discharge of a patient.
