Physical Therapist PT Home Health PRN

Core Responsibilities:

• Provides direct patient care, evaluates functional needs and outcomes, and consults with other specialists.
• Designs and implements a plan of care based on assessment and patient goals, incorporating participation by significant others as appropriate.
• Directs and supervises support staff to achieve functional goals.
• Documents evaluations, treatment goals, and plans, and maintains prescriptions and required signatures.
• Conducts therapy practice within defined standards of care.
• Provides direction and leadership to staff and promotes physical therapy in the community.

Qualifications:

• Graduate of an American Physical Therapy Association (APTA) approved physical therapy program.
• Current, unrestricted PT license valid in the state of practice.
• Minimum one (1) year of experience in a healthcare facility.
• Must have dependable vehicle, valid drivers license, and current auto insurance per state law.
• Able and willing to travel within coverage area, up to 50%.
• Ability to frequently lift, push, pull, and support patients and equipment up to 50 pounds.
