Home Health Physical Therapist (PT) – Full-Time / Part-Time / Weekend OnlyCentral Coast Home Health | 2 openings-Lompoc-Santa Ynez Valley and Santa Barbara proper.

Central Coast Home Health is a locally owned, fast-growing agency with an excellent reputation across the Central Coast. We are seeking Home Health Physical Therapists (PT) to provide high-quality, patient-centered care in 2 Santa Barbara County areas.

Responsibilities

• Provide in-home physical therapy services
• Develop individualized care plans
• Collaborate with interdisciplinary team members
• Document patient progress accurately and timely

Qualifications

• Active California Physical Therapist (PT) license
• CPR certification
• Valid driver’s license + clean driving record
• Reliable transportation & auto insurance
• Ability to work independently

Home health experience preferred; new grads welcome!

Pay

• Routine Visits: $70-$80
• Evaluations: $92–$106
• Admissions: $175.00–$200.00

    Actual pay rates vary depending on factors including type of visit, type of experience, location, and years of experience.
