PHYSICAL THERAPIST (PT) – PERMANENT (SNF)

Location: CA 90025
Start Date: ASAP
Schedule: Permanent (3 Openings: Ortho, Pelvic Floor, Lymphedema)

Compensation:

• $50 – $58 per hour

Position Summary & Responsibilities (SNF)

• Evaluate and treat SNF residents in assigned specialty area (Ortho, Pelvic Floor, or Lymphedema)

• Provide skilled therapy services and documentation

• Ensure Medicare/PDPM compliance

• Collaborate with interdisciplinary care team

• Maintain productivity and quality standards

Requirements

• Active Physical Therapist (PT) license

• SNF or post-acute rehab experience required

• Specialty experience in Ortho, Pelvic Floor, or Lymphedema preferred

• Knowledge of Medicare/PDPM and documentation standards

• Strong communication and teamwork skills
