Key Responsibilities
As a Physical Therapist at Canyon Springs Post Acute, you will play a vital role in helping residents regain strength, mobility, and independence.
Clinical Duties Include:
Conduct comprehensive evaluations and develop individualized treatment plans based on resident goals and clinical needs
Provide skilled, evidence-based physical therapy treatments in accordance with the California State Practice Act
Supervise Physical Therapist Assistants (PTAs) and support Restorative Nursing Aides to ensure continuity of care
Maintain timely and accurate documentation, including daily treatment notes, progress reports, and recertifications
Collaborate closely with nursing staff and the interdisciplinary care team; participate in care planning meetings and utilization reviews as needed
Educate residents and families on mobility safety, home exercise programs, and discharge planning
Recommend and coordinate durable medical equipment (DME) such as walkers, wheelchairs, and braces to support successful recovery
Qualifications
Current and unrestricted California Physical Therapist (PT) license required
Strong clinical judgment and documentation skills
Excellent communication and interpersonal abilities
Ability to motivate and engage residents throughout the rehabilitation process
Positive, team-oriented attitude with a passion for helping others regain independence
