$60 to $66 per hour | Full Time, Part Time, and PRN

At a Glance:

• Pay: $60 to $66 per hour based on experience
• Schedule: Monday through Friday, 8 hour shifts, flex options available
• Two Site Model: Fixed weekly schedule, for example Monday through Wednesday at Oakland Heights and Thursday and Friday at Lake Park, with no travel between sites in the same day
• Productivity Target: Averages around 83 - 85% but full details to be discussed during the interview
• Team: You'll partner with 2 PTs, 2 PTAs, 2 OTs, 2 COTAs and 2 SLPs
• Documentation: NetHealth and Point Click Care
• Parking: Onsite and street parking available
• New Grads Welcome: Pending California licensure considered, with structured mentorship

You'll work across two established skilled nursing buildings 2 miles apart on a predictable weekly schedule, with one leader and one set of expectations. Our residents come to us after hospital stays to rebuild strength, mobility, and the confidence to return home, and your evaluations set the course for their recovery. Full time, part time, and PRN schedules are all available, so you can build the week that fits your life.

What You'll Do:

• Complete PT evaluations and reevaluations, and build function based plans of care
• Treat residents individually and in groups to restore mobility, strength, and independence
• Supervise PTAs and guide rehab aides and clinical students
• Train residents and families on home exercise programs and safe use of assistive devices
• Contribute to care conferences, MDS input, and discharge planning
• Complete timely, accurate documentation

What You Need:

• California PT license, or pending licensure
• Doctor of Physical Therapy from an accredited program, or equivalent prior degree, and passed the NPTE
• Current BLS/CPR, or willingness to obtain
• Skilled nursing, post acute, or hospital experience preferred but not required
• Ability to safely assist with transfers, mobility, and repositioning
