Post Rehab is hiring a Physical Therapist (PT) for a full-time role in our skilled nursing facility. We are looking for a motivated clinician who is passionate about helping residents improve mobility, strength, safety, and overall function.

What You'll Do:

• Evaluate residents and create individualized treatment plans
• Provide therapy focused on mobility, balance, strength, pain management, and functional independence
• Collaborate with PTA staff, nursing, and interdisciplinary care teams
• Monitor resident progress and adjust treatment plans as needed
• Maintain accurate and timely documentation

Requirements:

• Active Physical Therapist license required
